Completing the Pre-Hire Questionnaire Steps (Candidates/New Hires)

Candidate Pre-Hire Questionnaire Experience:

***When completing the pre-hire questionnaire please note, NATIONAL ID NUMBER = SOCIAL SECURITY NUMBER (SSN)***

1. The e-mail employees receive will look like the below screenshot:

2. Please click on the link within the e-mail. You will be taken to a page that will load with 'Pre-Hire Questionnaire – Enter your e-mail address'.

NOTE: You must enter the e-mail that matches the e-mail address that the questionnaire was sent to in order for it to open.

3. Pre-Hire Questionnaire will load. Click Submit once completed. IMPORTANT NOTE: NATIONAL ID NUMBER = SOCIAL SECURITY NUMBER (SSN)

4. Once completed, the confirmation screen will show and you are finished:
